This position is for Full Time 32 hours

Position Summary:

Responsible for providing medically prescribed speech therapy in accordance with agency policies to clients in the home who have speech, language, and swallowing, and / or aural problems. Participates in developing and implementing the plan of care with goals and documentation appropriate to discipline guidelines.

Requirements

Essential Duties & Responsibilities:

  • Performs speech, language, swallowing and some aural evaluations, procedures, admissions, resumption of care and diagnostic tests on clients for whom these have been medically prescribed and establishes measurable goals.
  • Provides direct speech service to clients under physician’s plan of care. Records and reports to the physician the client’s response to treatment.
  • Maintains appropriate records on visits to clients, including evaluation and plan of care for speech clients.
  • Assists in coordinating speech therapy program with other services to family.
  • Demonstrates therapy for the family member or other responsible person who may assist the client with his speech, swallowing, and language rehabilitation.
  • Observes and evaluates the performance of those assisting in the speech program and provides guidance as necessary to assure a high quality of service.
  • Counsels the client and family members regarding the client’s speech, language, swallowing, and audiological disturbances.
  • Re-evaluates the client’s progress and therapy program making changes as necessary and under the physician’s direction.
  • Assists in referring clients to speech and audiology clinics and other community agencies.
  • Assists through individual and group staff education programs in preparing nursing and other staff to assist clients with their speech, language, swallowing, and audiological problems.
  • Participates in interdisciplinary conferences as required.
  • Submits clinical documentation in a timely manner according to agency policy.
  • Provides liaison contact and follow up with other community agencies.
  • Notes progress towards discharge planning. Communicates with physician and other disciplines regarding discharge planning.
  • Completes Oasis assessments accurately at appropriate time points.
  • Maintains all education required by licensure is current. Complies with OSHA, CHAP, State and Federal Standards.
  • Updates management regarding potential problems or concerns. Also responsible for consulting with clinicians or managers as needed.
  • Maintains a professional image.
  • Uses appropriate phone etiquette. 
  • Promotes a customer friendly atmosphere for all visitors and ensures client confidentiality at all times. 
  • Participates in accreditation program.
  • Performs other duties as assigned.

Education/Experience:  

MA/MS degree and graduation from an accredited school of Speech-Language Pathology required. Work experience of at least one year as a Speech- Language Pathologist.

Licenses, Certifications and/or Registrations:

  • Possess a Certificate of Clinical Competence in speech from the American Speech-Language and Hearing Association. 
  • CPR certification.  
  • Current drivers license with access to an auto and required insurance coverage.
