Job Description

                                            SUMMARY:

The Case Manager will conduct patient interviews and assessments, including level of care appropriateness, risks, and strengths. The Case Manager will engage patients in their treatment, continuing care planning and discharge planning. This role requires knowledge of and experience with community resources and referral systems and/or the ability to work in collaboration with community-based providers and agencies. This role requires basic knowledge of addiction, and mental health recovery processes, services, and systems of support. The Case Manager will promote recovery through role-modeling and respectful interactions with patients and colleagues.

ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:

  • Meet with individual caseload daily to assess further needs and progress in treatment, completing daily contact notes.
  • Conduct patient biopsychosocial and Columbia assessments within 48 hours of patient admittance. 
  • Prepare treatment plans and discharge planning in collaboration with patients and their identified supports.
  • Facilitate groups as assigned. 
  • Participate in staff supervision, in-service training, and continuing education necessary for certification requirements.
  • Manage caseload and track multiple required tasks using Electronic Health Record.
  • Communicate regularly with referral sources and advocacy groups.
  • Participate in continuing education necessary for certification requirements. 
  • As directed under supervision, interact with the financial and utilization management staff for reimbursement. 
  • Complete required documentation in accordance with Gosnold policy and regulatory standards. 
  • Abide by Gosnold professional code of ethics including those applicable to confidentiality, boundaries, dual relationships, and professional stature as a Gosnold representative. 
  • Communicate regularly with referral sources and advocacy groups.
  • Must possess skills in customer service orientation.
  • Perform all other duties as assigned.

MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job. 

  • Education: Bachelor’s degree in a healthcare or related discipline is preferred. 
  • Experience: Minimum one (1) year of experience in working with mental health, addiction and/or home health/hospice or related field will be considered for this position. Experience working in a team treatment approach in a multi-disciplinary setting. 
  • Skills: Excellent customer service and verbal and written communication skills. Computer literacy and working familiarity with technology.
