Job Summary:
The CCBHC Patient Specialist provides a full range of professional nursing services, including assessment, planning, implementation, evaluation, and coordination of patient care in accordance with individualized, team-based Comprehensive Care Plans. Under the direct supervision of the CCBHC Clinical Team Lead, the CCBHC Patient Specialist plays a critical role in supporting individuals as they navigate behavioral health, physical health, and social service systems. The CCBHC Patient Specialist will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.

  • Provide patient-centered care - identify, respect and care about patients' differences, values, preferences, and expressed needs; relieve pain and suffering; coordinate continuous care; listen to, clearly inform, communicate with, and educate patients; share decision making and management; and continuously advocate disease prevention, wellness, and promotion of health lifestyles, including a focus on population health.
  • Work in interdisciplinary teams - collaborate, communicate, and integrate care in teams to ensure that care is continuous and patient-centered.
  • Employ evidence-based practice - integrate best research with clinical expertise and patient values for optimum care and participate in learning and research activities to the extent feasible.
  • Utilize informatics - communicate, manage knowledge, mitigate errors, and support decision making using information technology.

  • Able to operate a computer and knowledge of Microsoft Office and be proficient in the use of electronic health record systems.

Job Requirements:
  • Bachelor’s degree in health-related field. Nursing or Social work degree preferred.
  • Minimum one (1) years’ experience in clinical care setting
  • Community Health Worker Certification or be able to obtain within first three months to six months of employment.
