Job Title: FCS Program Coordinator

Team: Navigation / Customer Care Support

Reports To: Director of Care Navigation

Hours: 40 hours per week

Compensation: Starting pay $24-26/hr depending on experience

Job Overview:


The FCS Program Coordinator supports the administration and implementation of the Foundational Community Supports (FCS) program while providing operational coordination between Customer Care, Navigation, and Finance teams.

This role ensures timely program enrollment, accurate documentation, compliance with program requirements, and coordination between Care Navigators, Customer Care staff, Finance, and external partners.

The coordinator manages application submissions, authorizations, billing preparation, and reporting while supporting Care Navigators in delivering high quality services to clients enrolled in the FCS program.

Job Duties:

  • FCS Intake & Enrollment Support
  • Collect required FCS intake information in Zoho
  • Obtain required FCS consent and signatures (in person when needed)
  • Verify Medicaid eligibility through ProviderOne
  • Review assessments and documentation for FCS eligibility requirements
  • Submit FCS assessments through the Wellpoint portal
  • Record submissions and tracking data in Zoho and program tracking spreadsheets
  • Track application expiration timelines and follow up with Care Navigators to prevent expired 

    Submissions

  • Complete new applications with clients when submissions expire
  • Application Processing & Authorization
  • Track pending submissions and missing information
  • Coordinate with Care Navigators to obtain required documentation
  • Record approval or denial responses in Zoho, Apricot, and tracking systems
  • Notify Care Navigators of application denials with guidance for resubmission
  • Assist Care Navigators in correcting and resubmitting applications
  • Open program enrollments for approved FCS clients
  • Document housing status and date of status for FCS enrollment

Reauthorization Management

  • Track FCS authorization end dates
  • Prompt Care Navigators to complete reauthorization requirements
  • Submit reauthorizations through the Wellpoint portal
  • Coordinate with Care Navigators to collect required documentation
  • Close program enrollments when authorizations end

Billing & Program Monitoring

  • Review case notes weekly to determine FCS billable activity viability
  • Provide weekly billable unit updates to Care Navigator managers
  • Meet with Care Navigators as needed to address documentation gaps
  • Submit monthly billing documentation to Finance
  • Assist in addressing claim rejections or corrections

Administrative Responsibilities

  • Submit required monthly program capacity reports
  • Attend monthly FCS program meetings and provider calls
  • Serve as a point of contact for Amerigroup and other FCS partners as needed
  • Maintain accurate program tracking tools and reporting systems

Transition Assistance Program (TAP)

  • Support administration of TAP funding when applicable
  • Formalize internal TAP processes and documentation
  • Train Care Navigators on TAP allowable expenses
  • Submit TAP funding applications

Team Support

  • Support Customer Care and Navigation teams with program coordination
  • Assist with intake questions and eligibility screening when needed
  • Provide guidance to Care Navigators on FCS documentation and workflow
  • Maintain accurate program documentation across Zoho, Apricot, and tracking systems
  • Participate in program meetings and internal coordination activities
  • Provide coverage and operational support for the Customer Care team during periods of downtime or staffing gaps, including assisting with intake, call coverage, and referral coordination
  • Perform other related duties as assigned to support program operations and organizational needs

Minimum Requirements:

  • Experience working in social services, housing programs, healthcare coordination, or case management support
  • Strong attention to detail and ability to manage multiple tracking systems
  • Experience with data entry and program documentation requirements
  • Excellent organizational and time management skills
  • Ability to communicate effectively with internal teams and external partners
  • Strong problem solving skills and ability to navigate program requirements
  • Comfort working with databases and client management systems (Zoho, Apricot, or similar)

Preferred:

  • Experience working with Medicaid funded programs
  • Familiarity with Foundational Community Supports (FCS) program requirements
  • Experience with housing programs or care coordination models
  • Bachelor’s degree in social services, public administration, healthcare administration, or related field
  • Experience supporting program billing, reporting, or compliance processes
