What You’ll Do

As a BHCM, you will serve as a critical connector between patients, providers, and behavioral health services—ensuring timely, coordinated, and effective care.

You will:

  • Coordinate and track behavioral health services across primary care, internal SVCHS programs, and community partners
  • Conduct behavioral health screenings (mental health, substance use, and social determinants of health)
  • Provide same-day consults in exam rooms to support primary care providers
  • Deliver brief, evidence-based interventions (e.g., motivational interviewing) to engage patients in care
  • Facilitate referrals and ensure follow-through with both internal and external services
  • Manage scheduling workflows to improve patient access to behavioral health services
  • Monitor and track patient progress using the electronic health record (EHR)
  • Support population health initiatives focused on behavioral health outcomes
  • Maintain strict adherence to confidentiality and compliance standards

What We’re Looking For

We’re seeking a professional who is both clinically aware and operationally strong—someone who can engage patients while navigating complex care systems.

Minimum Qualifications:

  • Bachelor’s degree in a human services-related field or equivalent experience
  • Strong interpersonal skills with the ability to motivate and engage patients
  • Ability to collaborate effectively within a multidisciplinary care team
  • Comfort with data entry, documentation, and basic data tracking/analysis
  • Working knowledge of behavioral health conditions and treatment approaches
  • Familiarity with community resources and referral processes

Preferred Competencies:

  • Experience with brief behavioral health interventions (e.g., motivational interviewing)
  • Understanding of integrated care or Federally QHC environments
  • Ability to communicate effectively with patients, families, and clinical teams
  • Strong judgment and ability to manage sensitive, confidential information
