Program: Young Empowered Parents (YEP) 

Reports To: YEP Director 

Status: Full-Time (40 hours/week) HYBRID in-person/remote work schedule 

FLSA Category: Non-Exempt 

Position Summary

Family Services of the Merrimack Valley is seeking a full-time, bilingual (English/Spanish) Case Manager for its Young Empowered Parent (YEP) Program. YEP is a community-based program designed to meet the diverse needs of pregnant and parenting young adults up to age 24 and their children. The program aims to improve outcomes for young parents, mitigate the adverse effects of early childbearing, and support families in achieving independence, self-sufficiency, and healthy child development. Key Duties and Responsibilities

  • Provide case management services to up to 20 pregnant and parenting young adults. 
  • Conduct regular home or community-based visits with clients. 
  • Facilitate and follow up on core and supportive services, including outreach, home visits, case management, individual coaching, parent education, and coordination of transportation and flex funding. 
  • Follow up on referrals through home visits, office visits, and phone communication on a weekly, bi-weekly, or as-needed basis. 
  • Conduct thorough strength-based assessments and develop individualized service plans for each family. 
  • Support participants in building self-sufficiency and navigating systems of care independently. 
  • Establish positive, supportive relationships with participants to provide counseling and guidance. 
  • Identify and coordinate services that benefit the family, such as housing, family planning, healthcare, nutrition, job training, and education. 
  • Communicate and coordinate with partner organizations, including schools, DCF, mental health providers, DTA, healthcare centers, WIC programs, Family Resource Centers, and housing agencies. 
  • Provide support services to facilitate access to resources, including transportation and flex funds. 
  • Collect and maintain program output and outcome data as requested by DPH. 

Requirements

Required Qualifications

  • Bilingual proficiency in English and Spanish (verbal and written). 
  • Associate’s degree in a human services field, or High School Diploma/GED with relevant experience working with pregnant and parenting young adults. 
  • Ability to conduct psycho-social assessments, develop individualized service plans, and monitor plan implementation. 
  • Experience supporting youth and young parents facing challenges such as domestic violence, substance abuse, housing instability, adverse health conditions, or risk of child neglect/abuse. 
  • Ability to maintain strict confidentiality. 
  • Exceptional written and verbal communication skills. 
  • Strong organizational skills with the ability to prioritize and multitask in a fast-paced environment. 
  • Reliable transportation for home visits and client transportation; valid driver’s license, clean driving record, and proper vehicle insurance required. 
  • Flexibility to work varied hours as required by program needs. 

Preferred Qualifications

  • Ability to work independently and collaboratively within a team. 
  • Proficiency in Microsoft Office (Outlook, Word, Excel) and database management; ability to learn new software applications. 
  • Experience working with diverse families and cultures, particularly the Latinx community. 
  • Knowledge of the Lawrence community is highly desirable.

COMPENSATION AND BENEFITS

  • This is a 40 hour per week, non-exempt benefited position with a pay range of $20-$22 per hour, commensurate upon experience.

  • This position is eligible for a $1,000 sign-on bonus. Earn the first $500, after successful completion of 30 days, then the final $500 after successful completion of 90 days!
