We have two (2) open positions, one (1) is for our Westmoreland office and one (1) is for our Gallatin office.

JOB SUMMARY:  We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our dedicated team at our Federally Qualified Health Center (FQHC). As an LPN, you will play a vital role in providing comprehensive healthcare services to our diverse patient population. Your expertise and commitment will contribute to our mission of delivering high-quality, accessible, and patient-centered care to individuals in need. PRIMARY DUTIES AND RESPONSIBILITIES:

  1. Patient Care and Assessment:
    1. Conduct initial patient assessments, including vital signs, medical history, and current health status.
    2. Administer medications, immunizations, and treatments following established protocols.
    3. Monitor patient progress, provide health education, and offer support and counseling as needed.
    4. Collaborate with healthcare providers to develop and implement individualized care plans.
  2. Clinic Support:
    1. Assist healthcare providers with examinations, procedures, and minor surgeries.
    2. Prepare and maintain medical equipment, ensuring their functionality and cleanliness.
    3. Maintain accurate and up-to-date patient records, documenting all interactions and interventions.
    4. Perform basic laboratory tests and specimen collection as required.
    5. Coordinate referrals to other healthcare professionals or community resources when necessary.
  3. Patient Advocacy and Education:
    1. Advocate for patients' rights, needs, and preferences, ensuring their voices are heard and respected.
    2. Educate patients and their families about health conditions, preventive care, and self-management techniques.
    3. Promote healthy lifestyles and disease prevention strategies within the community.
  4. Collaboration and Teamwork:
    1. Collaborate effectively with multidisciplinary teams, including physicians, nurses, and support staff.
    2. Participate in team meetings, case conferences, and quality improvement initiatives.
    3. Foster a positive and supportive work environment, promoting open communication and teamwork.

OFF-SITE WORK:

Although it is not typical, Off-Site Work could, at times, be required of this position.  This off-site work will typically include, but is not limited to, performing nursing duties (i.e. Vital Signs) at Health Fairs or other HOPE Sponsored Events and attending off-site meetings, trainings or conferences.

SKILLS/QUALIFICATIONS:

  1. Valid LPN license in Tennessee.
  2. Proven experience as an LPN in a healthcare setting, preferably in an FQHC or primary care clinic.
  3. Sound knowledge of nursing principles, practices, and procedures.
  4. Proficient in conducting patient assessments and providing direct patient care.
  5. Strong communication and interpersonal skills.
  6. Ability to work collaboratively as part of a team.
  7. Familiarity with electronic medical records (EMR) systems.
  8. Commitment to delivering culturally sensitive and patient-centered care.
