 Job Title: Patient Care Coordinator 

Full time: 40 hours per week. M-F 8a-4p

Job Summary: We are seeking a detail-oriented and organized Patient Coordinator to coordinate and manage the scheduling of infusion therapy services for our patients. The ideal candidate will possess excellent communication skills, a strong understanding of infusion therapy, and the ability to work collaboratively with healthcare teams to ensure timely and effective patient care.

Key Responsibilities:

  • Office Administration:
    • Answer phones
    • Receive and manage faxes and emails
    • Help facilitate communication of supply lists between RNs and pharmacies
  • Scheduling Coordination:
    • Manage and coordinate the scheduling of infusion therapy appointments for patients.
    • Collaborate with nursing staff and other healthcare providers to optimize appointment availability.
    • Help facilitate deliveries by the pharmacies to the patients
  • Patient Communication:
    • Communicate with patients to confirm appointments, confirm receipt of supplies and drug, and address any non-clinical questions or concerns.
    • Ensure patients are aware of their treatment schedules and any necessary follow-up.
  • Record Keeping:
    • Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
    • Document scheduling changes and communicate updates to relevant team members.
  • Maintain organized and complete patient charts, updated orders and document communication regarding patient care plans

  • Collaboration:
    • Work closely with infusion nurses and medical staff to ensure appropriate staffing levels for scheduled procedures.
    • Participate in team meetings to discuss scheduling challenges and develop solutions.

Qualifications:

  • Education:
  • High school diploma or equivalent required
  • College degree preferred
  • Experience:
    • Prior experience in infusion therapy or clinical scheduling preferred.
    • Familiarity with electronic health record (EHR) system, preferably Alayacare
  • Skills:
    • Strong organizational and time-management skills.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.

Working Conditions:

  • An office-based role.
