Ozarks Community Hospital (OCH), located in Webb City, MO has an opening for a Care Coordinator within our Medical Home department. This position will require some traveling to our surrounding clinics such as Carthage and Mt Vernon.

Under the Operations Director, this position will be responsible for ensuring that Medicaid beneficiaries are successfully enrolled in the MO DSS Health Home. They will track OCH's Health Home patient's ER utilization daily and follow patients' Medicaid eligibility/coverage. This position will serve as a point of contact for patients enrolled in the Health Home as well as assist self-pay patients in obtaining Medicaid coverage. They will work cooperatively with a patients' care team to maximize outcomes. Has the ability to perform duties consistent with Standards of Practice, goals and philosophy of Ozarks Community Hospital and the Employee Code of Conduct.

Requirements:

  • High school diploma or equivalent.
  • Minimum of one (2) year's medical office experience preferred.
  • Previous experience with Medicaid coverage preferred.
  • Completion of medical terminology course or equivalent experience preferred.
