Job Summary:   Responsible for providing nutrition care and education to Senior Total Life Care (TLC) participants in a home and community-based model of care.  Responsible for evaluating the participant’s nutritional status, development and implementation of nutritional care plans, assessment of nutrient requirements, recommendation of nutritional prescriptions, evaluation of participant tolerance, effectiveness of nutritional care, and communication of nutritional data and information to the Primary Care Providers and other health professionals. Reports directly to the Center Manager. 

 

Specifications

Education: Bachelor’s degree from ADA accredited institution with a major in nutrition.  Master’s Degree plus approved work experience preferred.

Experience: 1 years’ experience working with the frail elderly population.

Number and Type of Employees Supervised (optional): None

Licensure, Registry or Certification Required: ADA registered.  

Special Training: Requires ability to teach; analytical thinking; and ability to provide care to geriatric participants; knowledge of clinical dietetics.  Only act within the scope of his or her authority to practice.  Meet a standardized set of competencies for the specific position description established by Senior TLC and approved by CMS before working independently.

Immunizations:  Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact

Key Responsibilities: (*denotes an age-related skill or task)

  • Performs nutritional assessments and reassessments as indicated by nutrition standards.
  • Provides nutrition education to participants/caregivers as indicated and according to ADA standards. 
  • Works collaboratively with other team members.
  • Assists with coordination of 24-hour care delivery.
  • Documents participant changes appropriately in the medical record and communicates participant changes to team members as needed.
  • Updates participants’ care plans appropriately throughout the reassessment period.
  • Participates, collaborates, and contributes as a member of the Interdisciplinary team (IDT). Emphasizes teamwork and collaboration in all clinic and Interdisciplinary interactions.
  • Supports his/her Interdisciplinary Team and promotes unity among the team while interacting with the team, other co-workers, and/or participants. Supports team members and functions as a team player while providing that support and adheres to the “do no harm” team contract statement. 
  • Participates in participant care planning including the implementation of SMART goals/interventions for the participants’ care plans and enters all care plan information in a timely manner as per organizational protocols.
  • Accepts teammates’ differences in professional assessment findings, as they relate to participant care, and supports teams’ decisions while promoting a united team for the participants’ continuity of care and for staff development.
  • Supports team members and functions as a team player while providing that support and adheres to the “do no harm” team contract statement.
  • Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
