Shawnee Health is seeking an Adult Protective Services Case Worker for our Shawnee Health Carterville office. This position reports directly to the Adult Protective Services Unit Director.

 

Responsibilities

· Represents Shawnee Health Service in the most positive manner with prospective, former and current patients, clients, their families and the community we serve.

· Upholds the Mission Statement - The fundamental mission of Shawnee Health Service is to improve the health and welfare of southern Illinois and southwest Indiana residents through the promotion, development and administration of quality, preventive primary health and social services, while efficiently utilizing limited resources. This will include assessing and serving the needs of the underserved/vulnerable populations and designing programs and services, which are culturally and linguistically appropriate.

· Handles confidential information with tact and discretion.

· In conjunction with the supervisor, determine appropriate response time for each referral to the program.

· Initiates the assessment of crime victim's cases.

· Conducts face-to-face interview with the alleged victim.

· Determines the level of risk for the victim.

· Based on level of risk, directs and develops an emergency response plan to protect the victim. This plan may include a range of legal, medical, and social support services.

· Follows up on each case according to client need.

Acts as advocate on behalf of elderly crime victims and witnesses.

· Meets monthly with supervisor to review caseload status.

· Must have a valid Illinois driver’s license and automobile liability insurance on personal vehicles.

· Must have knowledge of the legal constraints of working with elderly crime victims’ cases including Guardianship and Orders of Protection.

· Experience required in crisis intervention, family counseling and mediation.

· Must be able to work independently and possess good decision-making skills.

· On-call rotation required.

· Each person employed as a APS Case Worker shall successfully complete IDOA Certification Training, Simulation, and Phase II Certification Training.

 

Requirements

· Master's degree from an accredited university or college in health, social services, social work, health care

administration, gerontology, disability studies, criminal justice or public administration;

or

· An active registered nurse license;

or

· A Bachelor of Science in Nursing (BSN) or a Bachelor of Arts (BA) or a Bachelor of Science (BS) from an accredited university or college in health, social sciences, social work, health care administration, gerontology, disability studies, criminal justice, or public administration and one-year experience in health or human services;

or

· An associate degree from an accredited program in health, nursing, social services, social work, health care administration, gerontology, disability studies, criminal justice or public administration and four years' experience in health or human services;

or

· An active licensed practical nurse license, with one-year experience in health or human services;

or

· Four years' experience with law enforcement or criminal investigations.

 

Starting salary – $45,400 – $48,165

Full-time employee benefits include:

· 12 vacation days earned in the first year

· 11 paid personal days/holidays per calendar year

· 6 sick days earned per year

· Retirement plan with 3% company contribution after becoming eligible in the plan (a matching contribution by the employee is not required to receive this benefit). Additional profit-sharing contributions to the retirement plan are possible. Employee deferrals to the 401(k) and Roth plan available
