WAGE RANGE: $56,000 - $64,000/annually
LOCATION: This role is primarily work from home but requires 5-15 client visits in Hennepin County per month.

JOB SUMMARY: Catholic Charities of St. Paul and Minneapolis is looking for caring, compassionate individuals to ensures older adults and individuals with disabilities live and thrive in the home of their choosing! This position provides trauma-informed, culturally responsive support to individuals and families served through Catholic Charities programs. The Social Worker I works directly with assigned clients to develop service plans, complete intake and annual assessments, and connect clients with appropriate community partners, resources, and Catholic Charities services.
The ideal candidate is dedicated to client success and social justice, skilled in documentation and advocacy, and able to navigate complex systems with care and professionalism. This role is responsible for maintaining accurate case records, adhering to funder and regulatory requirements, and providing education to clients and the community about available services and challenges.
