Working closely with the Interdisciplinary Team, the Home Care RN provides the necessary technical, organizational, and communication skills to efficiently assess needs and coordinate the delivery of home care and clinic services. The RN must demonstrate the knowledge and skills required to assess, plan care for, and provide service to frail elders according to assigned responsibilities and LIFE-Program standards.

We are seeking a Registered Nurse (RN) with at least one year of experience working with frail or elderly populations. The ideal candidate should have a working knowledge of the interdisciplinary model of care management, be proficient in computers, Microsoft Word, and Excel, and be able to relate well to seniors and their families. This role requires flexibility and the ability to deal with sensitive issues and facilitate problem-solving.

Overview

Registered Nurse R.N- $6,000 Sign-On Bonus (to be paid throughout the first year of employment).

LIFE Beaver County, PA - Full-time.

Rates start at $33 per hour.

Lutheran SeniorLife provides a comprehensive array of community-based health and social service programs throughout western Pennsylvania.

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

Under the supervision of the Director, Clinical & Home Care Services and working closely with the Interdisciplinary Team (IDT), the Home Care RN provides the technical, organizational, and communication skills necessary to efficiently assess the need for and coordinate delivery of home care and clinic services. The Home Care RN must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elders according to assigned responsibilities and LIFE-Program standards.


Qualifications

Registered Nurse Home Care qualifications:

  • A registered nurse must be a graduate of a school of professional nursing.
  • Minimum of one (1) years' experience working with a frail or elderly population. Working knowledge of the interdisciplinary model of care management. Experience working with cognitively impaired seniors is important. Must be able to relate well to seniors and their families to deal with sensitive issues and facilitate problem solving. Must be flexible and able to change easily.
  • Working knowledge of computers, Microsoft Word and Excel required.
