Unity House of Troy is seeking a compassionate and dedicated Case Manager to join our Community Residence team located on 8th Street. If you enjoy supporting individuals on a personal level and helping them navigate day-to-day routines, this may be the perfect opportunity for you.


Key Responsibilities

  • Foster a structured, supportive, and inclusive environment for residents.

  • Build positive, professional relationships with consumers.

  • Assist with daily living skills, including cooking, budgeting, and personal care.

  • Support residents with medication management.

  • Ensure the facility remains clean, safe, and well-functioning.

  • Provide supportive counseling and offer encouragement and positive feedback.

  • Transport clients and complete program-related errands as needed.

  • Promote a culture of belonging and respect.


Qualifications

  • High School Diploma or GED required.

  • Minimum 1 year of experience working with adults living with mental illness.

  • Valid NYS Driver License and reliable transportation.

  • Strong verbal and written communication skills.

  • Basic computer proficiency.

  • Ability to work independently and collaboratively as part of a team.
