We are currently hiring for a Client Care Coordinator in our Niagara Falls, NY Area! As a Client Care Coordinator at Venture Forthe, you will manage complex client scheduling needs, ensuring seamless care continuity by working closely with clinical staff, caregivers, and clients.

Work Environment: Office

Shift: Four (4) ten hour evening shifts (12pm-10:30pm)

Pay: Starts at $20/hour and goes up to $22/hour after three full months of employment

What you will do:

• Manage complex client scheduled for high-acuity cases, ensuring appropriate staffing and compliance with NYS Department of Health regulations
• Oversee last-minute staffing changes, call-offs, and emergency scheduling situations, proactively resolving conflicts to minimize client disruption
• Collaborate with clinical teams and nursing staff to align scheduling with client-specific care plans.
• Support Electronic Visit Verification (EVV) compliance, troubleshooting missed clock- ins, late visits, and system discrepancies
• Fill client schedules with HCSS and PCA services as directed by leadership.
• Assist with onboarding and continuous training of new coordinators, reinforcing scheduling protocols and company policies
• Address client or caregiver concerns related to scheduling, maintaining a professional and client focused approach
• Assist with day to day reporting
• Other duties as required

Requirements

QUALIFICATIONS

• High school diploma required; Associate’s or Bachelor’s degree in Healthcare Administration, Business, or a related field preferred.
• Experience with NYS home care regulations and LHCSA operations preferred.
