Behavioral Hospital of Bellaire is seeking a Step-Down Navigator to meet with all eligible patients to inform them about the benefits of stepdown services as part of their continuum of care treatment.  This role requires a deep understanding of the step-down process and strong coordination skills to facilitate seamless communication between departments, ensuring patients receive the appropriate support for their ongoing recovery.

Position Responsibilities include:

  • Meet with all eligible patients to discuss the benefits of appropriate programming options and guide them through the step-down process as part of their comprehensive continuum of care.
  • Assist in the development of strategies, goals and objectives related to the step-down process.
  • Collect and enter data specific to the step-down process, including eligibility, referrals, refusals, and reasons for refusal or acceptance, identifying and reporting on trends.
  • Coordinate with the Admissions Department, Clinical Services, Utilization Management, Business Office, and Outpatient Services on patient eligibility for programming options
  • Ensure that all aspects of the step-down process are followed: identification of eligible patients, insurance verification, treatment team approval etc.
  • Assists facility leaders in identifying trends to continue program expansion and reduce barriers to treatment.

Hours:

  • Monday through Friday 8:00 AM to 5:00 PM

Qualifications
