As a Hospice Home Care Liaison at MercyOne, you will be responsible for representing the Hospice's and VNA comprehensive services to community-based healthcare providers, patients and families and increasing revenue through admissions growth from both existing and new referral sources. Responsible for client concern resolution, interacting with Hospice and VNA staff to improve communication and increase understanding of referral source's needs and expectations. Establish and maintain communication internally and externally to optimize utilization of Hospice and Homecare services. Serves as a liaison between referral sources, Hospice Medical Director, Admission

Schedule:

  • Full time 40 hours a week
  • Day shift

General Requirements:

  • No Supervision: The job does not require the provision of guidance or supervision to
  • Limited:  Work requires limited responsibility for material resources.  Examples of resources could include supplies, equipment, inventories, small budgets, and other similar material assets.  The employee has a limited amount of control over these resources.  The cost of errors is also limited in terms of damage, waste or financial loss.  Problems associated with material resources are not complex.  The volume of resources may vary, but the level of difficulty in dealing with these resources is uncomplicated
  • Co-workers/Health System Employees, Outside Agencies/Other Health Care Providers, General Public/Visitors/Volunteers, Physicians/Medical Office Staff, Students/Interns/Residents/Outside Instructors, Patients, Families, and Significant Others, Vendors/Clients, Third Party Payors/Insurance Companie

Education:

  • Education:4 year college/university degree or equivalent experience
  • Field OF Study: Communications, Marketing, Sales, Public Relations, Nursing or Social Work
  • Special Training: Experience in sales/marketing Licensures/Registration Driver's License; Valid Auto insurance - Genesis minimum limits required
  • Experience: Less than 1 year experience required.
  • Interpersonal Skills: Interaction is with a variety of people inside or outside the organization.  Communications are often difficult or stressful in nature.  Contact with others involves complex, detailed and often sensitive topics.  The job requires a high degree of interpersonal skills to deal with a range of complicated problem situations.  Interactions involve earning the trust of children and adolescents, as well as an ability to communicate with them in a clear
